Contact Information How we will use this information: We will email you an electronic version of the finished book.  Also, we would like to place your contact information (especially your phone number so that your congressperson can contact you if they'd like to discuss the issues) along side/below your written contribution.  In the electronic format book we will post for free here on this site we will remove your contact information, except for your city/state and first name, so that you can not be personally identified... and so that you won't be bugged by telemarketers and the likes.

Family/Individual Photograph
To further legitimize these constituency books, and help our legislators visualize that these concerns are coming from actual people, their constituents who vote for them, we request that you submit a family or individual photo.  These photos will be printed in the book, next to your written contribution.  Please submit a large format photo and we can resize it to fit.  Please be sure, however, to only submit photos 2MB or less.

Written Contribution
The most important part of your submission is the written portion. This is what the congressperson will be reading and using to get an idea of where his constituents stand. Please make this thoughtful, constructive (be positive - not negative), and use clean/proper language. We would like the comments to be at least a quarter page in length. While we will edit the submissions for spelling/gramatical errors, or if they are too long please do your best to use correct spelling/grammar and keep them very descriptive but to the point.

Some things we'd like to see in the submissions (choose just one or two and focus on them):
- Your concerns with the HR 3962 Healthcare Bill congress is trying to pass. Click HERE to check the forum if you need help with this.
- Any personal stories that you have which will influence your congress person to vote NO on this bill.
- Suggestions and Proposals as to how our healthcare system can be improved.
- Your promise that you will not vote for any legislator who votes yes for this bill. Let them know you'll also spread the word.

We prefer a document upload in one of the following formats, but if you'd like to submit it via another format please just let us know by using the "Contact Us" menu above, and we will try to accommodate your needs.  Formats accepted: .odt .sxw .rtf .txt .doc OR .docx
